In order to join NACUSAC, please complete this NACUSAC Membership Application. You will need to:
a. First complete the form;
b. Second print two copies of the form keeping one copy for your records and another copy for you to send with your check for annual dues;
c. Third use the submit button to send an electronic copy to NACUSAC headquarters.
